Waiver.

1. Assumption of Risk

I acknowledge that my participation in axe throwing, rage room activities, splatter painting, spin art, golf simulator and other activities at UP 2 U entails known and unknown risks that could result in physical or emotional injury, paralysis, death, or damage to myself, to property, or to third parties. I understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of the activity.

The risks include, beyond others: cuts, bruises, muscle strains, twisted ankles, or other injuries resulting from throwing axes, breaking objects, or slipping and falling; exposure to paint fumes (though non-toxic); and potential contact with other participants.

I expressly agree and promise to accept and assume all of the risks existing in this activity. My participation in this activity is purely voluntary, and I elect to participate in spite of the risks.

2. Release of Liability

I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless UP 2 U from any and all claims, demands, or causes of action, which are in any way connected with my participation in this activity or my use of UP 2 U's equipment or facilities, including any such claims which allege negligent acts or omissions of UP 2 U.

3. Rules and Safety

I agree to abide by all the rules and instructions given by UP 2 U staff. I understand that:

• Closed-toe shoes are required for all activities involving heavy or sharp objects (Axe Throwing, Rage Room).
• Protective gear provided (such as safety glasses, suits) must be worn at all times in designated areas.
• No alcohol or drugs are permitted prior to or during axe throwing or rage room activities. Staff reserves the right to deny service to anyone suspected of being under the influence.

4. Medical Certification

I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating, or else I agree to bear the costs of such injury or damage myself. I further certify that I am willing to assume the risk of any medical or physical condition I may have.

5. Photography Release

I grant Up 2 U Entertainment, LLC the right to take photographs of me and my property in connection with the above-identified subject. I authorize Up 2 U Entertainment, LLC, its assigns and transferees to copyright, use and publish the same in print and/or electronically.
